Obesity drug reimbursement in Portugal: new SNS terms
October 26, 2026

A new Portuguese ordinance sets the terms for obesity drug reimbursement in Portugal, limiting state support to SNS patients followed by a hospital obesity team. Portaria n.º 464/2026/1 was published on 22 October 2026 in the Diário da República, Série I.
Terms of obesity drug reimbursement in Portugal
The state covers 15% of the public retail price, or of the reference price where one applies, under reimbursement band D (escalão D). Patients must be registered SNS users with an active clinical file under the national obesity pathway (PCIPO), followed by the hospital team of their local health unit (ULS) within a multidisciplinary programme covering nutrition, physical activity and psychological support. Only endocrinology, internal medicine or paediatric specialists on that team can prescribe, using an electronic prescription that names the regime.
Reimbursement runs for 12 months from the first dispensing, the minimum PCIPO period for judging whether treatment has worked. Renewal depends on a periodic review that the prescriber documents in the clinical record. Dispensing is limited to community pharmacies. The rules take effect 120 days after publication, on 19 February 2027, and cover only prescription obesity medicines that already hold a favourable reimbursement decision.
Why it matters
The ordinance builds on Despacho n.º 13066/2025 signed 30 October 2025, which set up the PCIPO and provided for an exceptional reimbursement regime for approved obesity medicines, to be set by ordinance. Access therefore runs through a hospital team rather than routine prescribing. The first performance indicator the Despacho lists is the share of adults reaching at least 5% weight loss after 12 months, the same window that limits funding. Portugal has restricted funding by clinical criteria before: its Briumvi decision for multiple sclerosis covers patients only after first-line failures. For market backdrop, see Mounjaro sales in Portugal. Which products qualify depends on prior favourable decisions, so the regime’s reach depends on that step.
