FDA Rare Disease Framework Initiative Would Tailor Review to Small Populations

The FDA rare disease framework initiative, announced on 14 October 2026, aims to replace a single review standard with one matched to the evidence that small patient populations can realistically produce. The FDA published a white paper alongside the announcement and will hold a public meeting on 28 October 2026 at its White Oak campus. The Reagan-Udall Foundation will facilitate the meeting.
What the FDA announced
The Rare Disease Framework Initiative is led by the agency’s Rare Disease Innovation Hub, working with the Office of the Commissioner, the Center for Biologics Evaluation and Research and the Center for Drug Evaluation and Research. The white paper, titled “A New Framework for Rare Disease Review,” says the goal is a process in which fit-for-purpose evidentiary expectations become the norm for all rare disease applications.
