Physical impairments, chronic conditions, and concerns such as device damage anxiety clustered together, widening disparities across every layer of the divide and contributing to elevated depressive symptoms plus reduced telemedicine effectiveness.

This pattern of digital exclusion among older adults functions as a structural driver rather than a simple technical gap. The systematic review followed PRISMA guidelines and drew from multiple databases through March 2025, applying rigorous bias checks via Joanna Briggs Institute tools to isolate age-specific findings. Results underscore how preexisting inequities amplify when healthcare shifts online without safeguards.

Policy Adjustments for Equitable Digital Care

Health economics evaluations must therefore model the combined effects of access, proficiency, and benefit-realization barriers when assessing digital interventions for aging populations. Reimbursement policies need built-in affordability measures, ongoing skills support, age-friendly interface designs, and continued nondigital options to prevent further marginalization. Future research incorporating longitudinal and intersectional perspectives can guide coordinated responses that address these overlapping determinants. The evidence highlights the necessity of preserving inclusive pathways while expanding technology, ensuring that digitized systems do not deepen divides for vulnerable older groups. Expanded analysis of intersectional factors, including gender and disability overlaps, reveals additional leverage points for intervention design that could improve health equity metrics across diverse national contexts.
